Tryout Information for November 20, 2011 @ 5:00-8:00pm
1. You must pay $14 to register the player/child online and join as a new member or renew as an existing member with AAU Sports and go to their website http://www.aausports.org.
Note use the AllStar Volleyball AAU CLUB CODE which is: TF4BFX.
2. You must pay $55 to register the player/child online and join as a new member or renew as an existing member with USA Volleyball Florida Region.
Pick AllStar Volleyball Club as your affiliation if you plan on playing with this club.
5. You must click on the handbook an read it to be part of this club!!!!!!!!!

3. Print, fill out, and get notarized....repeat...get notarized the 2012 Medical Release form go to http://www.usavfl.org click on forms and find the 2012 Medical Release form.
4. Copy the player/child's Birth Certificate. Repeat!! Just a Copy!
5. You must click on the handbook an read it to be part of this club!!!!!!!!!
6. Fill out the contact form on this website with AllStar Volleyball Club by clicking on
"contact forms" at the left hand side of the page to participate in the tryouts.
7. Bring $50.00** to the Tryout Outs. Checks made payable to ALLSTAR VOLLEYBALL CLUB.

**1st year Tryout Fee $50
2nd year AllStar Volleyball Club Player $35
3rd year AllStar Volleyball Club Player $25
Parents and Players will sign a contract commitment form and mail the contract with your 1st registration payment due Dec 1, 2011 if you are accepting a positioning on the team to the address below.
